Registration forms and information sheet has been attached below. Registrations can be done by one of the following methods after you have completed/signed the registration form;
1) Completed registration form along with CASH, CHEQUE or payment by CREDIT / DEBIT card at one of the Gurdwara’s
2) Completed registration form, CREDIT CARD payment form duly signed can me emailed to imahal@pusha.ca. One of the PUSHA official will confirm the registration by return email along with CREDIT/DEBIT CARD receipt.
